In consideration of my child being permitted to enter Once Upon A Dream, I agree to accompany, attend to, and monitor the play and activities of all the Participant(s) to ensure that all activities are appropriate and suitable for the age(s), weight(s) and ability of the Participant(s). I understand that play areas should be left as they were found, and I should clean up after my child. I assume all risks associated with my child’s participation and I understand that any potentially unsafe or dangerous behavior is prohibited, including but not limited to, running and climbing on top of the play equipment. I fully understand I am responsible for the safety of my child and all the risks associated with unsafe behavior that may cause injury.*

COVID-19 ADDENDUM: Once Upon A Dream has implemented new enhanced guidelines, to keep you, your family and our staff safe during this new normal; however, Once Upon A Dream cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending Open Play at Once Upon A Dream could increase your risk and your child(ren)’s risk of contracting COVID-19.*

COVID-19 SCREENING: All children, parents and staff will be screened prior to entrance to Once Upon A Dream. These screening questions are taken from the CDC, DSS & WHO recommendations. Along with these screening questions, the day of your reservation Staff will check child(ren)'s and adults temperature and check for symptoms at entry and ask if medications were used to lower the child’s temperature and if there are any household members with COVID-19.*

Have you or anyone in your household been directly exposed to someone with confirmed Coronavirus or a person under investigation? If yes, there is a 14-day waiting period for any one that has been exposed to or has tested positive for the coronavirus before they are able to enter, along with a physician’s note clearing the individual.*

We are a “well center” sick children or adults will not be permitted in. We encourage parents to be on the alert for signs of illness in their children and to keep them home when they are sick. If you or the child have flu-like symptoms such as fever, cough, chest pain or trouble breathing, we ask that you not come to the center, please go to the closest emergency room or urgent care center to be evaluated. We ask that if you do test positive for Coronavirus (COVID-19), and you have been in the center within the last two weeks, please let us know so that we can properly alert the staff and families who may have been exposed.*

ASSUMPTION OF RISK AND WAIVER OF LIABILITY: I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by visiting Once Upon A Dream and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Once Upon A Dream may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Once Upon A Dream employees, volunteers, and other participants and their families.*

Reservations are non refundable. Reservations start and end at reserved time (please be on time). If notice of cancellation is given 2 hours prior to reservation we may work with you to reschedule, depending on availability.*
